Liability Waiver & Release Form
I understand that participation in the USTA Hanmadang Tournament at Stanwood High School, Stanwood, Washington, involves physical activity and inherent risks, including but not limited to bodily injury, contact with other competitors, falls, and equipment-related injuries. I voluntarily choose to participate and assume all risks associated with my participation.
I agree to release, waive, discharge, and hold harmless USTA Martial Arts, event organizers, instructors, volunteers, sponsors, judges, and facility staff from any and all claims, demands, or causes of action arising from injury, loss, or damage, including those resulting from negligence, to the fullest extent permitted by law.
I understand that I am solely responsible for any medical expenses, treatment, or damages incurred as a result of participation.
I grant permission for emergency medical treatment if deemed necessary and certify that I am physically able to participate in this event.
I also grant permission for photographs or video recordings taken during the event to be used for promotional or educational purposes without compensation.

I, the parent or legal guardian, consent to my child’s participation in the USTA Hanmadang Tournament and accept full responsibility for any injury, loss, or damage sustained. I agree to all terms of this waiver on behalf of myself and my child.
